Do you know a person, a team or a club who deserves recognition for what they have achieved this past year? A relative, a neighbour, someone at your club? Tell us about them using the form below — it takes about ten minutes, and there is no PDF to print, scan or post.

Before you start

Achievements must have taken place between 1 October 2025 and 30 September 2026. Anything outside that window cannot be considered.
The nominee must either live, or have lived, in the District of Mansfield — or play their sport here.
You will need a photograph of the nominee. A picture straight off your phone is fine; it is resized for you before it is sent.
A short video of them in action, around 30 to 45 seconds, helps the judges. Upload it to YouTube, Google Drive, Dropbox or WeTransfer and paste the link — please do not try to attach the video itself.
Please tell the person you are nominating them.
Junior awards are for those aged under 18 on 31 October.
